SEMINAR WAIVER FORM

TERMS AND CONDITIONS

Please refer below to the terms and conditions regarding the purchase of Seminar Services provided by Dale Halaway and Seminars That Inspire, LLC.

Engagement:
Thank you for accepting our offer to present our Seminar Services to you. By registering for this seminar, you agree that these Terms and Conditions form a contract between you and us.

Investment and Payment:
Your payment of the Investment to us entitles you the right to:
(a) a seat at the Location during the Seminar Hours as agreed on the Event Details; and
(b) to receive a copy of the Materials (subject to the Intellectual Property clause below)

Refunds:
We may cancel the Seminar Services for any reason by written notice to you. We will also refund you the Investment you have actually paid to us within 7 days of giving you notice of cancellation. We shall have no further liability to you in respect of the cancellation

We acknowledge that you shall not be entitled, and shall not claim a refund once you have registered.

Transferring Programs:
If you advise us by notice in writing 5 days prior to the Seminar for which you are registered that you are unable to attend the Seminar, you may utilize your total investment and allocate it to another one of our Seminars within 90 days of the original program date.

If you do not advise us by notice in writing within 5 days prior to the Seminar for which you are registered and you are unable to attend, you may utilize your total investment, less a 10% administration fee, and allocate it to another one of our Seminars within 90 days of the original program date.

Intellectual Property:
We have all right, title and interest in all intellectual property used by us in performing the Seminar Services (subject to the rights of the presenters) and all intellectual property we may develop as a result of the performance of the Seminar Services.

In performing the Seminar Services for you, we do not transfer title to you for any intellectual property. All such intellectual property remains with us.

You may not:
(a) use a recording device or record the event
(b) alter any of our intellectual property or the Materials
(c) copy or reproduce the content of the Seminar Services or the Materials
(d) sell the Intellectual Property or the Materials or supply the Intellectual Property to third parties

Disclosures:

You acknowledge:

(a) that while Dale Halaway and the staff/associates of Seminars That Inspire are Transformational Life Coaches dedicated to providing self-improvement training & advanced philosophies, they are not licensed health care providers

(b) that the Relevant Persons do not represent themselves in any way as having this education, training or licensing, and as such, they do not provide health care services or health education to prevent, diagnose, or treat any physical and/or mental disease, disorder, or condition

(c) that you have not been coerced or threatened by anyone into taking this Seminar Service and that yourself, your heirs, successors, executors, administrators or assignees hereby release and forever discharge Dale Halaway and Seminars That Inspire, LLC, or any of their Associates from any and all causes of action, claims and demands for any damages which heretofore may be sustained as a consequence of attending this Seminar Service

(d) that in consideration of your participation in this Seminar Service, you agree to fully release and hold harmless, Dale Halaway and Seminars That Inspire, LLC and all its principals, directors, shareholders, employees, agents, promoters, independent contractors, sponsors, volunteer assistants, associated coaches, vendors (including without limitation the hotel or venue where the Seminar Services are conducted) and others associated with the Company from any claim or liability whatsoever and for any damage or injury, personal, emotional, psychological or otherwise, which I might incur as a result of my voluntary decision to participate in activities associated with the Seminar Services
